Court FormsSeveral forms are available from the District Court for a small fee of 25 cents to $1.00 per form. There is a limit of 10 forms per request. All forms are approved by the State Court Administrative Office. This page offers a web link to the Michigan Supreme Court. You may download (free of charge) approved forms from that site. Complete the forms and bring to the Court for filing. Because some filings require multi-part forms, it may be necessary for the clerk to make additional copies from your original at a fee of $1.00 per page.
